To request research from materials at the Heritage Quest Research Library, fill out this form and mail it to the address shown above. One research request is allowed at a time. Please provide as much information as possible in the lines below. For research within the Library, the charge is $15.00 per hour, minimum one half-hour ($7.50), plus postage and $.25 per page for copies.
